• SMART Backend Services with a JWT. Request exactly the scope the vendor gives you.
• The FHIR base URL includes a tenant user id that is returned in the token response — read it from the token before building data URLs.
• Token requests can fail transiently; a few retries are normal. Tokens observed lasting 2 hours.

Extraction approach

There is no population-level search, so the patient list has to be enumerated:

1. Walk Patient?name=<letter>&_count=500 for each vowel, de-duplicating as you go and recording progress per letter so the walk can resume.
2. Run a per-patient cascade for clinical resources.
3. Extract text from PDF documents (DocumentReference) where results such as depression screens are only recorded in documents.
